    The Work Health and Safety Regulations 2011(Cth) (WHS Regulations) require certain duty holders to give Comcare copies of health monitoring reports and notices of asbestos removal work, asbestos fibre levels and emergency demolition of structures containing asbestos, that meet certain criteria.



 

  • Asbestos Removal Licence – Supervisor Nomination

    This application form is to be used in conjunction with the Asbestos Removal Licence application form and is approved by Comcare for the purposes of administering the Work Health and Safety Act 2011 (WHS Act).

    This form is provided by Comcare for the purposes of Regulations 376, 413 and 442 of the Work Health and Safety Regulations 2011 (WHS Regulations). Its purpose is to assist persons conducting a business or undertaking (PCBU) to meet their obligation to give copies of relevant health monitoring reports to Comcare.



 


 


 


 

  • Notification form—Major hazard facility

    This form is for notifying Comcare of Major Hazard Facilities (MHFs), propose MHFs, existing MHFs and changes to existing facilities that will require notification.



 

  • Notification of an incident

    The Work Health and Safety Act 2011 (Cth) (WHS Act) requires a person conducting a business or undertaking (PCBU) to ensure that Comcare is notified immediately after becoming aware that a notifiable incident has occurred which arises out of the conduct of its business or undertaking.



 

  • Notification of demolition work

    This notification form is provided by Comcare to assist a person conducting a business or undertaking (PCBU), or an emergency service organisation (ESO), who proposes to carry out demolition work to meet their obligation to provide written notice to Comcare as required by regulation 142 of the Work Health and Safety Regulations 2011 (Cth) (WHS Regulations).



 

  • Notification of emergency demolition involving asbestos

    This notification form is provided by Comcare to assist duty holders to meet their obligations by providing written notice to Comcare—as required by Regulations 454 and 455 of the Work Health and Safety Regulations 2011 (WHS Regulations)



 


 


 

  • Notification of licensed asbestos removal work

    This notification form is provided by Comcare to assist licensed asbestos removalists meet their obligation to provide written notice to Comcare as required by regulation 466 of the Work Health and Safety Regulations 2011 (Cth) (WHS Regulations).



 

  • Notification of removal of a worker from lead risk work

    This form is provided by Comcare for the purposes of Regulation 415(2) to assist persons conducting a business or undertaking (PCBU) to meet their obligation to notify Comcare if a worker is removed from carrying out lead risk work under subregulation 415(1).

  • Provisional improvement notice (PIN) form

    A health and safety representative (HSR) may issue a provisional improvement notice (PIN) if they reasonably believe that a provision of the WHS Act or Regulations is being contravened, or has been contravened in circumstances that make it likely that it will continue or be repeated. This form provides a PIN template that is approved for use by Comcare.
