OHS Incident - Notification and reporting

If, at or near a workplace at which an undertaking is being conducted by the operator, there is, arising out of the conduct of the undertaking:

(a) an accident that causes the death of, or serious personal injury to, any person; or

(b) an accident that causes the employee who performs work in connection with the undertaking to be incapacitated from performing work for a period prescribed (currently 5 successive days or more); or

(c) a dangerous occurrence

the operator must, in accordance with the OHS(MI) regulations, give to AMSA's OHS Inspectorate  [link] notice of , and a report about, the accident or dangerous occurrence as follows:

  • within 4 hours an Incident Alert - using Seacare Form   - AMSA Form 18) [pdf 93Kb];  and
  • within 72 hours an Incident Report - using Seacare Form -   AMSA Form 19) [pdf 71Kb] 

 

Penalties are prescribed for non reporting.
