Installation
|
- Insufficient space allowed for access to machine
- Equipment incorrectly installed (e.g. not stable or level on base)
|
- Check adequate space has been allowed for access during operation and maintenance
- Ensure equipment installed, tested and demonstrated before accepting delivery
- Ensure equipment is installed close to power points to reduce the need for extension leads
- Where possible consider using classified waste removal systems in preference to a shredder

Staff training
|
- Workers are unable to use advanced functions correctly causing malfunctions
- Technical staff are unfamiliar with equipment to maintain it properly
- Loose clothing / long hair becoming entangled in equipment
|
- Ensure that installer provides operator training for workers following installation
- Check that instruction manual is provided and located adjacent to the equipment
- Arrange training for technical staff to be responsible for maintaining equipment and providing assistance to workers
- Ensure signage is displayed reminding workers to tuck in loose clothing and to put hair up when using equipment

Equipment malfunction
|
- Workers poke fingers and other objects into equipment in attempts to clear paper fragments
- Using an unsuitable grade of copy paper
|
- Ensure workers understand instructions for dealing correctly with paper jams
- Switch off power supply at wall before attempting to clear a jam
- Place notice on / near equipment with phone number to call for assistance with re-setting machine
- Check that copy paper used conforms with the machine manufacturers paper specifications
