This form provides a way for individual health and safety representatives (HSRs) to register their details with the Seacare Authority and the Australian Maritime Safety Authority (AMSA). Registration is voluntary.

By submitting your name and contact details below, the Seacare Authority or AMSA may contact you via email to advise you of upcoming events, as well as seek your input and feedback on various matters.
