The role of workplace rehabilitation providers
Workplace rehabilitation providers play an important role in the return to work process. They also support Comcare’s objective of improving recovery and return to work for employees and promoting the health benefits of good work.

Workplace Rehabilitation Provider Management Framework
All Comcare-approved workplace rehabilitation providers are required to comply with Comcare’s Workplace Rehabilitation Provider Management Framework.
The Framework sets performance and compliance requirements and expectations for providing workplace rehabilitation programs and services under the Comcare scheme. It consists of:
- Operational standards
- Criteria for approval and renewal
- Workplace rehabilitation provider conditions of approval (PDF, 137.1 KB)

Applying for approval as a provider
Only approved workplace rehabilitation providers can deliver rehabilitation services in the Comcare scheme.
Before applying you should review our Workplace Rehabilitation Provider Management Framework to check eligibility criteria, standards and performance expectations.
For new applicants, there is a legislated non-refundable application fee of $2,000.

Consultant training
Workplace rehabilitation provider consultants are required to complete Comcare training which consists of an online eLearn and an interactive virtual session.
Forms
These forms can be used for processes that relate to workplace rehabilitation providers.
- Rehabilitation assessment (PDF, 238.1 KB) This form documents a determination to arrange an assessment of an employee’s capability to undertake a rehabilitation program.
- Rehabilitation assessment examination (PDF, 248.7 KB) This form documents a determination that an employee is required to undertake a medical examination as part of a rehabilitation assessment.
- Rehabilitation program (PDF, 552.9 KB) Use this form to document the agreed rehabilitation program for an injured or ill employee.
- Rehabilitation program alteration (PDF, 241.9 KB) Use this form to document changes to an agreed rehabilitation program once it has started, to achieve the original agreed goals. The changes may include altered timeframes or additional costs or services. Use the form each time a program is altered. If the overall program goals change, the program should be closed using the Rehabilitation program closure record form.
- Rehabilitation program closure record (PDF, 412.8 KB) Use this form to notify Comcare when a rehabilitation program closes. It includes reasons for the closure and program outcomes.
- Suitable duties (PDF, 108.3 KB) This form helps identify suitable duties after an injury has occurred.
- Work trial agreement (PDF, 130.2 KB) An agreement aims to secure a common understanding and commitment to the work trial.
